The School Based Health Centers serve the health care needs for enrolled students in the Pulaski school district. The centers are state funded health care facilities, and are an extension of Northern Oswego County Health Services Incorporated. Care is provided to enrolled students with either insurance billing, or with no fee should the pupil not be covered by an insurance plan.
The School Based Health Centers will be staffed by: Celeste Carnes, RN, FNP, and Sharon Ellis, RN, FNP. Our collaborating physician is Jay Chapman, MD. Mental Health Counselors are also available.
The center's hours of operation follow the school schedule. When the centers are closed, patients are encouraged to call and use the Pulaski Health Center at 298-6564 for their health care needs. You can visit their website at http://www.pulaskihealthcenter.org.
Among the many services available from the School-Based Health Centers, with Parent/Guardian permission are:
• Diagnosis and treatment for illness
• Allergy shots
• Comprehensive Physicals
• Prescriptions - Called to patient chosen pharmacy
• Referrals for counseling
• X-Ray scheduling
Enrolled students are usually seen the same day as an appointment is made. Please call the School Based Health Centers at 298-2696 (high school) and 298-2570 (elementary school) to enroll, make an appointment, or with any questions. Your concerns will be addressed promptly.
