Welcome to the Nurse's Office

There is a Nurse's Office at Lura Sharp Elementary School and the Middle-High School. Each school also has a School Based Health Clinic (SBHC). The school nurse is available to assist all students in the school. The SBHC is a full-service health center operated by ConnextCare. The SBHC primary care provider is available to treat any students enrolled in their program.
If your child is sick, they must stay home if they have a temperature over 100°F or are having vomiting or diarrhea. Your child cannot return to school until they have been fever free (without fever-reducing medication) and have not had vomiting or diarrhea for at least 24 hours.
If your child needs prescription or non-prescription medication during school hours, please refer to the school’s Medication Procedure. Students must not carry medication in school. The only exceptions are students who have written permission to self-carry and administer emergency medications. Student physicals are mandated by New York State for all new students and students in grades Pre-K or K, 1, 3, 5, 7, 9 and 11. Documentation of the exam must be completed by the provider on the Required NYS Health Examination Form. Students enrolled in the SBHC may have their physical completed in school. If your child has his or her physical with another provider, please submit a copy of the Required NYS School Health Examination Form to the school nurse. Dental Health Certificates are also requested for students in a grade in which physicals are required. These forms are available on this page.
Physicals are required for any student wishing to participate in interscholastic sports. Interval Health History must also be completed by a parent/guardian through the FamilyID sports clearance site within 30 days of the start of each sports season. If your child is in a grade in which physicals are required, his/her weight status will be reported to the New York State Weight Status Survey (BMI Survey). Only summary information is sent; no names or individual student information is sent. However, you may choose to have your child’s information excluded from the survey.
