School-based Health Center

  • The School Based Health Centers provide prompt, convenient, quality healthcare to enrolled students in the Pulaski School District. The centers are state funded health care facilities, and are an extension of ConnextCare. There are no out-of-pocket charges for services provided at the SBHC. We will bill most insurances and we accept what we receive as payment in full. There are some services, such as prescriptions, lab work, x-rays and referral appointments, which you would be financially responsible for. If you do not have medical coverage, ConnextCare has Outreach Coordinators that can assist you in obtaining insurance coverage.

The center's hours of operation follow the school schedule. When the centers are closed, patients are encouraged to call and use the Pulaski Health Center at 315-298-6564 for their health care needs.
Among the many services available from the School Based Health Centers, with Parent/Guardian permission are:
          • Diagnosis and treatment for illness
          • Immunizations
          • Allergy shots
          • Comprehensive Physicals
          • Counseling
          • Prescriptions - called to patients-chosen pharmacy
          • X-Ray scheduling
          • Lab work
          • Referrals

To enroll your child, simply complete an enrollment form for each child. Forms are located at each SBHC and online at or the main office.

Enrolled students are usually seen the same day as an appointment is made. Please call the School Based Health Centers at 315-298-2696 (high school) and 315-298-2570 (elementary school) to enroll, make an appointment, or with any questions.
